Client Record Charts: Consultation Forms, Pre-Session Questionnaire Forms & Treatment Notes
To provide holistic therapies, and give you the best and safest treatment possible, you will complete an online form or we will complete a hard copy consultation form at the start of your first appointment.
Consultation forms and Treatment Notes are stored only in hard copy paper form (we print your electronic forms).
Newly filled out consultation forms are put in a separate folder and locked into the filing cabinet at the end of each working day.
To provide you with the most effective service, we may offer (specifically when you opt in for PSYCH-K® sessions) an optional pre-session PSYCH-K® Questionnaire Form to be filled by you before you come in for your first session. You are in no way obliged to fill out this form, and the form exists to solely focus your mind on what you wish to address during your session, so as to use time during the sessions more effectively. If you choose to use the form, with your verbal or written consent, this form will be sent out to you via email and, only if you choose to, you may fill this form out and email it back to us – in which case it will be printed out and added to your hard copy client record chart. Alternatively, you can print out or write your answers by hand and bring in the hard copy with you to your first PSYCH-K® appointment. This copy will then be added to your securely stored client record chart.
All client record charts, including all hard copy forms, are securely stored in a locked press to which only we have access. Each client’s records are stored as long as each client is attending sessions or workshops with us. If and when the client no longer wishes to return for sessions, the records can be destroyed within 3 weeks upon request, alternatively, they are stored for up to 7 years (to comply with our insurance policy) in a locked safe after which time they are safely destroyed in compliance with the current Data Protection Regulations.
We do not remove client records from storage, nor share the information with any third party.
